Project BOOST (Better Outcomes for Older adults through Safe Transitions) is a Society of Hospital of Medicine initiative focused on improving the care of patients as they transfer from hospital to home. BOOST is led by a national advisory board of recognized leaders in care transitions, hospital medicine, payers and regulatory agencies.
